چکیده

M itral regurgitation (MR) is a common form of valvular disease1 and its severity is associated with excess morbidity and mortality from left ventricular (LV) remodeling, left atrial remodeling, pulmonary pressure elevation, and progression to left and right heart failure.2-6 Current American Society of Echocardiography (ASE) Guidelines7 propose vena contracta (VC) width as a simple semi-quantitative marker of MR severity. The VC width is defined as the minimal color Doppler diameter at the junction of the flow convergence acceleration zone and the turbulent regurgitant jet just distal to the regurgitant orifice (Figure 1). One limitation of this simple measure is that the VC width is only a single tomographic slice of a three-dimensional flow event. Three-dimensional vena contracta area (3D-VC area) is a novel measurement made possible by 3D color Doppler imaging. Here we review the methodology and clinical validation of this new measurement tool for the quantification of MR severity.

Comparison of vena contracta width by multiplane transesophageal echocardiography with quantitative Doppler assessment of mitral regurgitation.

Mitral regurgitation (MR) severity is routinely assessed by Doppler color flow mapping, which is subject to technical and hemodynamic variables. Vena contracta width may be less influenced by hemodynamic variables and has previously been shown to correlate with angiographic estimates of MR severity. This study was performed to compare mitral vena contracta width by multiplane transesophageal ec...

Assessment of Mitral Regurgitation Severity by Doppler Color Flow Mapping of the Vena Contracta in Dogs

BACKGROUND Although Doppler color flow mapping is widely used to assess the severity of mitral regurgitation (MR), a simple, accurate, and quantitative marker of MR by color flow mapping remains elusive. We hypothesized that vena contracta width by color flow mapping would accurately predict the severity of MR. METHODS AND RESULTS We studied 80 patients with MR. Vena contracta width was measu...

Severity of aortic regurgitation assessed by area of vena contracta: a clinical two-dimensional and three-dimensional color Doppler imaging study

BACKGROUND Quantitation of aortic regurgitation (AR) using two-dimensional (2D) echocardiography, including vena contracta width (VCW) measurement, is still challenging. Three-dimensional (3D) echocardiography can directly measure the vena contracta area (VCA), regardless of the rheological characteristics. We intended to assess the possibility of 3D vena contracta area (3DVCA) as well as 2D ve...

Analysis of procedural effects of percutaneous edge-to-edge mitral valve repair by 2D and 3D echocardiography.

BACKGROUND Analysis of procedural effects in patients undergoing percutaneous mitral valve repair (PMVR) using the edge-to-edge technique is complex, and common methods to define mitral regurgitation severity based on 2-dimensional (2D) echocardiography are not validated for postprocedural double-orifice mitral valve. This study used 3D transesophageal echocardiography (TEE) to determine the fu...

A Mathematical Evaluation of Mitral Regurgitation Severity with EROA

It is to be investigated and verified that vena contracta width (VCW) obtained from color Doppler images and effective regurgitant orifice area (EROA) obtained from the VCW is capable of quantifying the severity of mitral regurgitation (MR). A mathematical expression has been developed between VCW and EROA. Vena contracta width is an efficient method for grading the severity of MR, but it is ve...
